Before You Start: Version and Platform
PAPERHEAD lists 25 achievements on its full-game Steam store page. The Xbox list recorded by Exophase also contains 25 achievements, worth a combined 1,000 Gamerscore. The descriptions and scores below belong to that Xbox list; 1,000G is an Xbox total.
Use this list for the full PAPERHEAD game, Steam app 2680280. PAPERHEAD EP.0 is a separate prologue under app 3008200, with its own 11 Steam achievements. A shared character name does not establish a shared unlock route. The PAPERHEAD walkthrough coverage explains the version and route information available for the full release.
Missable Achievements and Unlock Coverage
The database descriptions identify the listed objectives, but they do not supply a verified route for every unlock. Some state an action directly; others are short hints. Treat the wording as a checklist of published descriptions rather than a sequence of steps to follow through the campaign.

Character and Rank Challenges
Character Unlocks
Five descriptions name characters to unlock: Super Bandage Boy, Jeb The Scientist, Lost Soul, Golden Buddy and Drone 0. Their presence in the list confirms those named objectives. Where each character is found and what triggers each unlock: Not yet confirmed.
Full-Level Character Challenges
The list separately requires finishing all levels as Super Bandage Boy and as Jeb The Scientist. Unlocking either character and completing that character's full-level objective are separate entries. How character completion is tracked, and whether it can be combined with other challenge progress: Not yet confirmed.
PERFECT and HARDCORE Objectives
The Xbox descriptions distinguish a first PERFECT rank on any level from PERFECT on every level. Completing all levels on HARDCORE difficulty is another listed objective. Rank thresholds, difficulty unlock rules and a verified route for combining these challenges: Not yet confirmed.
Lore and Other Discovery Objectives
The Xbox list includes “Find ALL lore!” for 100G. That establishes an all-lore objective. The total lore count, item locations and whether all lore equals all secrets: Not yet confirmed.
Other descriptions mention discoveries such as petting the chill boy, while several are cryptic phrases. Their wording does not identify a chapter or location. The PAPERHEAD secrets and collectibles guide separates the known lore objective from the paper clip resources described in launch coverage.
PAPERHEAD EP.0 Uses a Separate List
The official EP.0 Steam achievement list contains 11 objectives. They include finishing EP.0 once, unlocking PROFI and Super Bandage Boy, finishing as Super Bandage Boy, and earning a first ULTIMATE rank on any level. The full-game Xbox rank descriptions use PERFECT instead.
Keep an EP.0 checklist labeled as EP.0. Whether its character unlock instructions work unchanged in the full release: Not yet confirmed. Likewise, full-game achievement completion should not be inferred from an EP.0 save or its achievements.
